Monolith's BLOOD was and still is my favorite horror themed game. The level design is fantastic. The "Easter Eggs" or "Nods" to horror films within are hilarious. The game is utterly unsurpassed by any other in re-playability. I should know, still playing since 1997 on a daily basis. I literally spent years trying to make it run every time a new operating system came out that wasn't DOS friendly. Thanks to GOG that is no longer necessary. It is fun, scary, and challenging. BLOOD for my money all the way to Hell and back.
Lots of games come to mind, but I will go with Alan Wake, the intro, story, music and especially the end are worthy of a novel and movie. better even :)
I cannot recommend Call of Cthulhu enough. First, the story-line was written by the same people that write for the Tabletop RPG Call of Cthulhu. Second, it actually uses proper role playing elements that effect how the game plays out! You literally won't see some hidden items unless you upgrade your Spot Hidden ability. This effects how you play, what you are able to do in the game, and how the game ends as well. Your experience is largely reliant on how you upgrade your character.

This game was hugely misunderstood when it came out... Many thought it would be a jump-scare fist person shooter or something. But, that goes directly against the game description itself! It's a narrative horror experience which includes many elements of different genres... So there is a part of the game where you shoot, but it is only brief. There are elements of survival horror, but only in certain parts. Some puzzle solving also. All the while trying to balance your sanity and abilities.

I loved the story. Being a fan of H.P. Lovecraft's works I can say that it closely resembles the atmosphere's he paints himself, and the dread of the Old Ones is there with you for the entire game.

Wonderfully done! I hope this developer decides to make more Call of Cthulhu TRPG inspired games!
Recently ´ve played Phantasmagoria 2: A puzzle of flesh, it´s a kind of horror game more psychological, a deep game that dig in the mind.
The scariest horror game I've seen is Simon The Sorcerer 3D from former Horror Soft guys. I cannot even handle myself into buying it because of shivering after looking on screenshots. These guys know how to create pristine horror, give them credit for that.
I recommend the first FEAR game and its expansions, it's a scary and exciting experience, and the best part: the shotgun. In second place, I think both Resident Evil 7 and 8 are good scary games, and the first-person perspective adds more emotion. Thank you for the contest!
The first time I perceived the xenomorph in Alien Isolation was the scariest moment i experienced in any form of entertainment media. At the point that I felt like running towards it just to be killed and end the tension I was experiencing until then. After fighting that urge away, it was definitely the scariest and most challenging ride I experienced in any video game.

I recommend Blood II and its. add-on. I think it tends to be overlooked. I loved the classic Blood but the newer graphics in Blood II really look cool. It has a great horror movie atmosphere throughout.
My recommendation is Dead Space!
Amnesia: The Dark Descent - One of the few scary games I played. I watched Day 9 scream his way through the first two hours. Played the game myself and then watched Day 9 scream his was through the rest of the game.

I'd recommend Carrion by Devolver Digital for a good indie horror game. Who wouldn't want to be the monster causing the horror rather than be the victim trying to avoid or fight to survive the monster? The animations are satisfyingly creepy and really conveys the terrifying characteristics of the monster very well in my opinion. You can almost feel the slime and texture of the monster's tendrils as they grab your hapless human prey for a quick snack as you sneak, slither, & rampage through dark air ducts and underground labs. Why feel the horror when you can be the horror?
