The top left thingy makes me thing about red cells... So my first though was "Bad blood" but it's rather unlikely because then it would be hard as hell guess therefore I'll go with "Two Worlds: Tainted Blood"

I looked up "video game with macrophages" and came up with something called Immune Attack.

Hint: the game's visual style is completely different. This is just my rendition of a scene in a game, just like grimwerk did several times already (except his drawings don't look like a lefty using the mouse in the right foot's).
