How about these?

In the Wizardry series, teleporting into solid rock is game over for the entire party; the entire party is lost forever and deleted from disk. (The Wizardry Gaiden series, at least 3 and 4, is a bit more lenient; the party is instead killed and teleported back to town, making it at least possible to recover from death.)

In Wizardry 3, to my understanding, the developers did something *really* nasty: there is one square of solid rock that you can just walk into. Doing so will cause your entire party to be lost, no teleport needed. Do you think that's fair?

Wizardry Gaiden 4, which I am playing through now, also has a trap like this; you go through a door, and your party is now in rock and everybody's dead. I was really not expecting the game, which had been easy combat-wise up to that point, to pull such a nasty trick on the player. Do you think that's fair?

In Elminage Gothic, enemies can cast a spell called Miracle which has a chance of teleporting your party away. On at least 2 occasions, I had my party teleported into rock; in one case, it was an ambush, so my party was wiped out before I could enter a command. Do you think that's fair? (At least Elminage Gothic doesn't auto-save, so you can reload if that should happen to you, and I recommend doing so even if you normally like to play under iron man rules.)

The earliest funny one I saw was Commander Keen's supposed demise when hit in the first game. I don't really know what's supposed to even be happening in the animation. His mid section kind of disappears and then he flies of screen?

The first gruesome one was being cut in half in PoP, which I found somewhat disturbing as a kid. I felt my toes curling up in my shoes whenever I slowly approached those blades :P

If I had to pick a favorite it would have to be the lava death in PoP2. One time in high school a friend of mine was visiting, a really big and chubby rugby player with a great sense of humor. We were playing PoP2 and my mom called me to help her with something in the kitchen. I'll never forget hearing from my room the sound of the prince screaming as he fell into the lava, followed by my friend chuckling like an idiot. On repeat as well, because he kept loading saves so that he could jump into the lava over and over while waiting for me XD

Actually maybe this doesn't count but I like the cut-scene in Warcraft 2 where the human sneaks up and kills the orc then launches the catapult it was guarding at the orc zeppelin *boom.* When I was younger it brought a tear of joy to my rosy cheeks. I like all the cut-scenes but for some reason that one stays with me the most, well that and the one where the human king is calling for his guards before the orcs pay him a visit. Muhahaha
