As far as "who the hell thought of that?" promotions for games, Capcom scattered fake, severed arms and legs throughout a city to "promote" Resident Evil 5. It was incredibly tasteless and resulted in complaints when random people in the city found what looked like severed limbs. Updated to add which game was being "promoted."
