Does Nancy the Happy Whore need a Desura account to work, because I saw "third party DRM : Desura" ?

You are confused. Its ShinyLoot page says:

DRM/Protection:DRM-free
Third Party Keys:Desura

That means you can download it via ShinyLoot DRM-Free and in addition you receive a Desura key to activate it on Desura if you so desire.

You can download the installer directly from ShinyLoot and you get an additional Desura key.
