The main character is diagnosed with the mental disease shared by the title, which causes loss of reality. He's in a mental hospital, and he's just gouged his eyes out. However, that's not stopping him from seeing insane s**t. In fact, this mod's pretty messed up.
You need Half-Life 2 (and maybe the episodes) to play this. I highly recommend it, as well as Dear Esther, another Half-Life 2 mod that focuses on a haunting narrative rather than shootie-shootie.
EDIT: Needs Episode 2 of Half-Life 2.

No thanks, I like my videogames to be places I escape TO, not FROM.
That said, I don't think I ever really thought about the number of mods available for HL2 that I could download and play. I may have just found myself a deep well of cheap entertainment... so thank you for bringing this to my attention.

No problem.
I just finished the mod (no, it's not too short, I played it for a bit before posting), and HOLY CRAP. This has to be one of the most amazingly terrifying experiences and stories I have ever borne witness to. If this were a commercial project with just a bit more polish and less fighting, this would be close to Game of the Year for me.
EDIT: Also, I ended up enabling God mode because the combat is frustrating.

I hope this will make me shit in my pants. I remember when EA was like, DEAD SPACE IS SPOOOOOOOOOOOOKY and I basically went through the game without so much as wet pants. Most of the time, I find modders and indie devs to actually, you know, try to make me scared. I'll try this... soon.

Yeah, I know. I wish these guys would've shipped out the adult diapers instead of F.E.A.R. 2, because while that game was sort of scary, this game is insane. Also, this game has an extremely original story. I never knew this medical condition existed, and now I'm scared shitless that it may happen to me someday.

Dear Esther is very spooky. Nothing ever happened in my playthrough, but it used several tricks to make you think someone was on the island with you. Also, the story is very open to interpretation about what's going on, since it never gives you all the facts.
I downloaded this the day it came out, and I try to start it, and it says in Steam it needs 3 hours to start the game.

I HATE backtracking. It is interesting though, combat is stupid. They could almost have gotten away without enemies.
Finished. Pretty damn good, quite a few bad design decisions thou. I know creator was probly aiming for confusion and fear, so we can feel as Christopher felt, well... kinda, however... Well... Whoever thought of spinning instakill TVs and screen going white while you really needed to focus on jumping should get punished. I didn't find combat so annoying, thou - it was actually pretty easy
How do you apply the mod after download? Is it like fallout 3 modding in which you extract the files into the data folder?

You take the folder and put in your Steam folder through Steam/SteamApps/SourceMods. Just stick the folder in SourceMods, restart Steam, and it'll appear on your games list.

You just extract the mod into SourceMods directory, restart Steam and launch it as any other Steam game
