So I recently completed a massive run of Mount & Blade, from lowly peasant to great king.
(Insert "Started from the Bottom" song here)

However, the gameplay of the game is starting to get, well, stale after a week or so...

So, I'm looking for a new joy!
I'd like another sandbox game with rpg elements sprinkled in!
It doesn't have to be medieval either!

Any suggestions?
War of the Roses?
Give Space Rangers 2: Reboot a try... I think you'll like it.

A really crazy suggestion:
Boiling Point - Road to Hell
I think it wasn't ever released digitally but the sequel (Xenus 2 - White Gold) is available on GamersGate (haven't ever played that one, though).

Boiling Point is kind of a blend of Stalker and Far Cry. Huge jungle environment with a few towns/settlements, several factions you can support or fight, a decent amount of vehicles (ranging from cars to heilcopters), LOTS of weapons etc.. It has its quirks (huge amounts of them) and the plot and dialogue are very trashy but it's actually surprisingly good for something that got forgotten so fast. As far as I can tell a lot of problems were caused by the high system requirements back then which shouldn't be an issue now (although I haven't checked how the game runs on Windows 7 yet). Anyway, that's the first thing that came to my mind when I heard "sandbox with RPG elements".

The developers also created a scifi RPG called Precursors. Which as far as I can tell is very similar in terms of gameplay but set in a scifi scenario (haven't played that one yet, either).

ggf162: War of the Roses?
Not a sandbox. Doesn't even have proper singleplayer mode.

...modded Mount & Blade?

Since my gaming computer has been out of order for months now, I can't give you an opinion on many of them, but The Last Days mod for vanilla M&B is, in a nutshell, an awesome piece of work. Give it a try.
There seems to be a lot of interesting settings for M&B & Warband mods, such as 16th century south-east Asia, the Napoleonic wars, Viking Scandinavia, pre-1066 Britain, Star Wars, the Far West...

As for F4llout's suggestion, Boiling Point is the archetypal over-ambitious, under-funded game. You may like it, you won't fall in love with it.

Boiling Point is available from the Atari digital store:

I also recommend this one with the caveat that it's extremely buggy, and due to certain game design decisions some of the sandbox elements are somewhat lacking. But it has a certain charm to it that makes it one of my favorite games, and it does give players a great deal of freedom.
I haven't finished the other games yet, but it's a shame the developers are now working on casual games instead of polishing these games. =/

Yes, this is a good game. I need to play it again some time.
Oh, Deep Shadows. It's a shame, they have so much squandered potential. They could have polished their games more, or continued working on another, but I guess this trilogy is all we get. They used to be rather overpriced on GamersGate, but you can White Gold and Precursors for $10 each on GamersGate, which is less than I paid on sale. Unfortunately, they still haven't quite scrapped the DRM yet, but cracking them was a simple enough task.

I didn't know until now that Boiling Point was available digitally. It seems they're still doing that god-awful Digital River nonsense. I'm not interested in temporary downloads, and I too many others are, either. Thankfully, you can get a physical copy of Boiling Point for about the same price. I really like Deep Shadows' games. They're awesome. They are very rough around the edges, though.

Xenus II is arguably the best of the three. It feels like they had less and less development time with each one. Precursors is much more outlandish, but the weapons and environments aren't quite as nice as the more grounded tropical theme, and it feels like the vast majority of their planet designing efforts went into Goldin. Some of the planets are more disappointing than others, but some of them are still really neat. It's just that it would have been nice if all of them didn't feel considerably more rushed than Goldin. Precursors is really neat, but it was even more over-ambitious than Xenus II and it's more apparent.

Boiling Point was pretty damn buggy, White Gold improved a lot, but there were certain aspects of the original that I liked more. I wouldn't really compare any of them to Mount & Blade, I haven't really found anything else quite as adept at consuming hours like seconds so mercilessly, but they're pretty awesome. I would say start with Xenus II: White Gold and try The Precursors after that. I can't really recommend one over the other, they're great in their own ways, but Xenus II felt a lot more polished with one nice world instead of a couple of mostly small planets. Precursors lets you get drunk and high off your ass on Tatooine and pilot your own ship across the galaxy, so try not to miss out on that one.
If you still want to get some gameplay out of M&B, try one of the many mods for it. There are many great quality mods that changes up the original quite a bit, and you are sure to find one that will suit your tastes. Between M&B and Warband, mods range from adding a bit to the native game to complete overhauls of the Lord of the Rings, Star Wars, Wild West, etc.
You could try some of the sandbox space games, like the X series.

Yeah, that is the problem with Mount & Blade. The game gets stale after you become King. Still a great game though.

You can always play this if you seek something similar. Same old but also very different.
