Actually the "failure" happens earlier and it's connected to your choice whether you accuse Stennins of poisoning or not. Personally, I think it's a game bug that it's getting marked as "failed" after it was marked "successful".
ACCEPT IT AS IT IS.
THERE ARE SOME QUEST WHERE YOU ACCUSE THE WRONG MAN, OR LET THE REAL CRIMINAL DIES, OR COULDN'T MANAGE TO GET ALL THE CLUES IN TIMELY MANNER, YOU FAIL THAT QUEST.
IT IS A GOOD RPG THAT MAKES ALLOW A QUEST TO FAIL !!!!!
