As usual, am I missing something? This is in White Orchard. I hear it in certain area but can't track it down.

You get that if you go to areas that are nowhere near the quest you have targeted.

If you are working on a quest in Velen and you wander off to the Skellige Isle you will probably hear the cuckoo telling you that you are crazy for being there ... that it is nowhere near what you are looking for.
