I had this same extremely annoying issue.
Go to C:\GOG Games\The Witcher 3 Wild Hunt\bin\x64
Delete d3d11.dll

(stolen from another forum)
If it doesn't work just verify/repair the game in GOG

How much system RAM do you have available? I had the same problem. I had 8GB but I had partitioned 3GB into RAMdisc. I eliminated that and everything has been good since. W3 really does need 6GB min to run properly, at least on my system.

Also investigate your power settings. Several times I was throttled by my cpu because of it.
