I'm having a bucketload of trouble getting Planescape Torment to load and run properly with the widescreen mod.
I've followed the tutorial on here and it seemed to work perfectly except the scrolling went nuts and kept moving at top speed to the upper left. I pulled out the joypad I had plugged in but that didn't seem to help so I give the system a reboot. Loading up PST now I'm greeted with:
"An assertion failed in F:\Torment\source\chitin\ChDimm.cpp at line number 787" (on the system I'm using now F is not my CD drive and on my laptop I don't even HAVE an F drive but have the same error)
When I was playing this on vista some time ago using just the widescreen mod, the game ran pretty well (though with broken spell graphics and a small interface) but crashed every time I quit. Never did figure out why but I have a suspicion that this error might be caused by the same thing.
I'm discounting the usual drivers / compatibility mode / direct X problems since aside from the input issue it seemed to run flawlessly the first time (the initial dialogue with Morte was exactly as it should be)
Anyone managed to beat this exact issue? 5 shiny rep points up for grabs
This question / problem has been solved by lukaszthegreatimage
I can run the game widescreen without any mods. Are you sure they're necessary?
Seems like it has problems finding the disc or something. Check the Torment.ini file and see where it looks for the discs and the folder on the hard drive.

Do you have the jewel case or big box? Jewel case, for reasons I forget, crashes more. Also, this mod might have been designed for one and not the other.

Dumping the content from all four discs into the install directory then (as stated) modifying the Torment.ini file to point to your install directory for all five(?) discs may help.
I had this exact same problem.
I fixed it.
I have no idea how.
I am going to try and remember : ).
Ah!
Remembered. In my case, it was because I chose resolution 1280*800.
And this res was not natively supported by my graphics card and monitor.
So I went to drivers panel and set this resolution manually.
Now it works like a dream.
Found even more detailed help (courtesy of rpgcodex):
How I could tell my res was non-standard, when I got to Start->Settings->Control Panel->Display in the "Settings" tab theres a slider with screen resolution. When I change that slider around there isn't a 1280x800 displayed as an option (this is your problem).
Because I have an NVidia card/driver the fix is this:
1. Click the advanced button on the settings tab you're in
2. In the advanced menu, find the otpion on one of the tabs of "Start the NVIDIA Control Panel" and click it
3. In the Advanced Settings, go to "Manage Custom Resolutions" or something like that depending on your driver version.
4. There should be a checkbox along the lines of "Allow modes not exposed by the display" and check this box.
5. Now go back to your display settings resolution slider, 1280x800 should now be an option and this mod should work!

Necessary to play, no. Necessary to enjoy on the size of the display I'm using, yes sadly

Done that, no help

A kind of cardboard sleeve, its the 4 disc version anyway but the mod details imply very strongly that they're identical save for the application of the patch

Did that as part of the initial mod (as I said I followed those instructions I posted the link to)

This was one thing I didn't think to try before but sadly its made the same difference as the other options.
Looks like I'm just going to have to live on memories of the game being awesome, its more trouble than its worth at the moment.

did you try to install every mod listed in the tutorial you posted?
widescreen mod worked for me without any glitch.(i had to copy the cds to my hd. im surprised that tutorial does not say that you must do that. ) but i have superior system to you (XP) so maybe it is time for an upgrade Alias?
The widescreen mod alone worked on my vista lappy but the graphics were messed up so I wanted to use the mod that repairs it but thats where the problem starts.
Superior system? I've tried it on both my vista laptop and XP media centre pc
Try copying each entire disc into it's own folder instead of combining them like the guide said. Also try pointing "disc 5" at a folder as well (even if that folder doesn't exist).
No idea if that will work, but it seems to be the only thing I've done differently to my install.
Ghostdog is a Codex regular and I'm sure he knows a lot about that kind of thing. You could try asking there, I am sure he will be happy to help.
Qwinn used to post there too, but unfortunately the KKKodex didn't scale to his level.

only the spells which took a quarter of the screen. is that really a problem?
edit:
google came up with this:
http://www.gamebanshee.com/forums/planescape-torment-19/fixing-an-assertion-failed-809-a-66243.html
I bought the 2CD version and whenever I tried to run the game the "An assertion failed.... 809". After some tweaking with the Torment.ini, I've managed to fix the error, and would like to share the method.
All you have to do is open up Torment.ini (in the directory where you've installed the game).
Under [Alias], I originally had:
CD
CD
CD
CD
HD0:=C:\Program Files\Black Isle\Plane
Then I changed the lines to:
CD1:=D:\CD2
CD2:=D:\CD2
CD3:=D:\CD2
CD4:=D:\CD2
HD0:=C:\Program Files\Black Isle\Planescape
[*Note* I installed the game in the folder "Planescape" instead of the one originally intended by the setup.exe (which was like "\Planescape -TM"). I did this because at first I thought the name of the folder was causing the problem, but it wasn't. Also please note that the pre-edited Torment.ini had wrongly placed the installed directory (on the last line). ]
Assuming D: is your CD drive and C: is the HDD where you've installed the game. The last line consists of the location where you've installed the game. Originally, when the game was first installed, the location was incorrectly adressed (i.e. "HD0:=C:\Program Files\Black Isle\Plane") in the Torment.ini, after it was fixed it resolved the assertion failure.
By typing the first 4 lines I fixed the "Unable to detect Disc 2" error, which came after I fixed the assertion failure problem.
Also, by copying the content of CD2 into a folder and change the directory of the first four lines under [Alias] to the new directory, you might be able to play the game without the CD. This is a wild guess, I've never tried it, if anyone tries please let me know the results.
I hope this is able to help someone out there who are having trouble playing the game.

For all Infinity Engine based games (BG series; PST; Icewind Dale series) -- also make sure to go to your DirectX Settings and TURN OFF The Graphics option of Hardware Acceleration .
Well I tried one last reinstall on my vista lappy following the steps exactly (just like the last 2 times on this and my other computer) but this time it worked pretty much perfectly. I think I'll write it off to the lady of pain mazing whatever bug I was encountering.
Now all I need to do is find time to play it in amongst writing for my short story class, coding for my IT project, coming up with ideas for a story and game mechanics for ERNIE and playing with my shiny new PS3.
I think lukaszthegreat gets the 5 rep points because even though I solved the problem myself, he posted the most text.
