Same here. Just started this afternoon, after it crashed. I was playing the game, still in act 1 and the game crashed, and a weird crash at that. Just a crash to desktop, no message saying the game crashed, nothing. Just closed.

After I started back up, none of the voice-over or music would play, however the ambient sounds were working fine. The above mentioned fixes have not given me any love either.
I get this problem very occasionally too. I suspect it's to do with an action being performed during dialogue; as mentioned, other sounds are heard fine, but dialogue - and sometimes ambient sounds - cut out. Reloading a save normally works fine for me.

I had this problem too, voices completely disappearing for whatever reason.

It was fixed by loading an earlier save and then loading my current one, and bam, voices were back.

However not long ago I started the game up again to play, and suddenly I've got absolutely no sound at all, no idea why. I'm not using one of those G35 headsets, my sound configuration is set to headphones, not 7.1, but alas, still no sound.

Sound card is a Sound Blaster X-Fi Titanium.
I also had it and I own a X-FI extreme music or something...
If you ask me, I think it started when I alt+tab the game and kept it there for a while. I had to load a save game from behind to get it back. Maybe it sound stupid and without any connections but I;ve never tried to reenact the problem to be sure that the alt+tab is the starter.
I have the ASUS Xonar D1 soundcard and have had no problems with other games. I tried the start-new-game-load-current-save fix but that did not help. I wish the devs. would just take a look at this.... :(

If you have a 5.1 or 4.1 sound system...

Workaround #1, partially solves the problem: Set speakers to 4.1 (from 5.1), makes the problem almost not appear. With the exception of Loredos base, especially at the end of act 1, where you should switch to...

Workaround #2, which solves the problem completely (for me at last): Set speakers to 2.0. Of course this somewhat diminishes the experience since TW2 features some good surround sound. So I only suggest this drastical action when you're running into this audio issue frequently.

Xorbius: Same happened to me. X-fi gamer card. Played fine and then went to load and all sound except voices play now. I also had another glitch where it wouldn't let me save for like an hour, I just kept playing hoping it would fix but it didn't and once I went out of town, I got in a battle and I couldn't even draw any weapons no matter what I tried, so I died and since I couldn't save and it didn't auto-save, I lost over an hour of play time.
That second bug is literally the very same thing that I experienced myself, right after talking to Loredo at his house (I lost an hour of play time because of that). And now, I find myself with that first bug, no voices or ambient sounds at all! I tried loading a bunch of previous saves, but same issue, they're all without any sound...
I didn't see anything about it in the changelog, but I really hope the upcoming patch will solve those two issues.
The new patch did not change anything regarding audio and its hard to find any other people bejond this thread's that have the same problem.
I wish the devs would just frikken read this topic...
the only new problem I'm having in this regard is that the music in and around the Kaedweni camp skips or doesn't play at all

also when running through puddles there is no longer the sound of water splashing at Geralt's feet, it just sounds like he's running on dirt now

I've had this problem as well, but I've had an even more annoying problem in both Witcher games; I get static and crackling noises during both game-play and cutscenes. I can't figure out what causes this but it gets to the point where I end up skipping the cutscenes and turning my volume down.

Has anyone else had this problem and/or any ways to resolve this?

Same problem here. I lost all the voices. Sound effects, music and ambient are still there. Screwing around with the saves and starting new games doesn't seem to be working for me.

ATI HD4850 512 MB
Vista Home Premium
Abit P-35P onboard audio (Realtek HD)
I found another workaround. When you start the game go start a new game and skip all the cutscenes till when you speak to Vernon Roche in the prison. Just choose one of the dialogue options and when geralt is done talking switch back to main menu and load one of your saves. You will now have voices again.

You have to do this everytime you start the game though.

EDIT:
On advice of the witcher 2 support team I updated my nvidia drivers. When i updated to 275.33 the voices came back :). So try updating those drivers
