Story:Cadaver: The Payoff is an add-on that requires the original disc to play. It continues the story and offers four more isometric levels of fiendish puzzles. After defeating Dianos, Karadoc returns to the inn only to find his employers gone, and every living person within the city either dead or transformed into a monster. Seeking the cause behind this, the dwarf fights his way to the local temple, and finds the answers - and his money - deep down in the crypts. Gameplay is similar. Cadaver: The Payoff has only four levels, but they are significantly larger than those of the first game, and the second level has a large number of monsters and scant possibilities to heal.Show more

Unlike earlier survival horror games that focused on protagonists with combat training, the main character Harry Mason is an average man. The gameplay consists of combat, exploration, and puzzle-solving. The controller vibration is used to indicate Harry's heartbeat and will vibrate on low health. The player must regularly enter an inventory screen to check Harry's health, use items, and equip different weapons.

In the game's trailer, it is revealed that since then, both men have recovered and Jericho has escaped to San Francisco, while Tanner has pursued him there. Jericho is shown being transported in the back of a prison truck, but manages to escape with a vial of acid hidden within his mouth. He overpowers the guards, and hijacks the truck. Tanner and Tobias witness this from Tanner's car, pursuing Jericho as he causes havoc on the streets of the city. Tanner ends up in front of Jericho in an alleyway, and gets pushed in front of a tractor trailer. A hard crash occurs, putting him into a coma. The game will take place in Tanner's coma dream
